The people we support are the focus of everything we do and our 'person-centred' approach starts from the moment you apply to use any of our services.

We want to make sure we can meet your needs and that you will be happy with us. At the same time there are lots of practical and financial concerns and, of course, paperwork!

You need to read these two key documents:

Grange Application Form

Guidance on funding of placements

If you are interested in The Grange for a potential placement in any of our services, you are welcome to come and visit on any of the following occasions:

Visitor Afternoons

Come and look around the site, meet our staff and the people we support in their own environment. See for yourself what goes on in Skills & Activities and visit Supported Living and Residential Care where staff will explain how they can support you. Upcoming Visitor Afternoons are: 11 April 2018 and 29 August 2018. Contact Reception to book a place. Tel: 01372 452608 or email: [email protected]

Summer Fun 2018

Summer Fun sessions are a chance to come to the Grange for as many or as few days as you like and take part in crafts, sport and leisure activities, cookery and outings. You can meet new people, try new things and really get a feel for life at The Grange. Next year's dates are 9 July - 17 August 2018.

Application Form

Fill out The Grange Application Form and attach an up-to-date Review or Care Plan.  Email both documents to [email protected] or post to Referrals and Outcomes Co-ordinator, The Grange, Rectory Lane, Bookham KT23 4DZ

Home visits and tasters

Once we receive an application form and supporting evidence, we will either come for a home visit and/or invite you to a taster day. If we feel your application is suitable, we will then invite you to a 2-week residential assessment for applications to our Supported Living or Residential Care services or a 3-day assessment for our Skills & Activities service.

Final decision

Following the assessment, our staff will decide whether a permanent placement is right for you.

Waiting list

If a suitable place is not available immediately, your name will be added to our waiting list. This list is actively managed by our Referrals and Outcomes Coordinator and we will keep in contact with you and let you know as soon as we have an opening.
